A thorough history and physical examination will be done. The diagnosis and treatment options will be discussed. Patients can print, fill out, and bring these forms to their first visit.

You will be seen by a Physician at every visit.

What to bring to your appointment:
Please bring all pertinent prior records: reports of imaging studies like X-rays, CT scans, MRI. Please bring a list of all medications including any over the counter medications, herbs, and vitamins. Make a list of all your allergies. A copy of your current insurance card and a valid photo ID will be required.

A 24-hour notice is appreciated if you are not able to keep your appointment
