What to bring to your appointment:
Please bring all pertinent prior records: reports of imaging studies like X-rays, CT scans, MRI. Please bring a list of all medications including any over the counter medications, herbs, and vitamins. Make a list of all your allergies. A copy of your current insurance card and a valid photo ID will be required.
