Being well-prepared for your appointment will ensure that your pain specialist has all of the needed information to provide the best possible care for you. Payment Options

Pain Specialty Consultants, P.A desires to provide treatment to all who need help with pain. Payment is expected at the time of visit. Co-pays will be due at the time of the visit. Unpaid deductibles may have to be paid. As a courtesy, we will file claims with your insurance. Returned checks will incur an additional charge of $30.00 which should be immediately paid.


Appointments

What to expect at your first visit: A thorough history and physical examination will be done. The diagnosis and treatment options will be discussed. Patients can print, fill out and bring these patient forms to their first visit.

What to bring to your appointment:

Please bring all pertinent prior records, reports of imaging studies like X-rays, CT scans, MRI. Please bring a list of all medications including any over the counter medications, herbs, and vitamins. Make a list of all your allergies. A copy of your current insurance card and a valid photo ID will be required.

Cancellations

A 24-hour notice is appreciated if you are not able to keep your appointment.
